1. ASSUMPTION OF RISK
I understand that participation in martial arts training and related activities involves inherent risks, including but not limited to physical contact, strenuous exertion, falls, strikes, throws, joint manipulation, and the use of training equipment.
These activities may include, but are not limited to:

Technique training and drills

Controlled sparring with other students (with required protective equipment)

Forms, self‑defense, and conditioning

Demonstrations, tournaments, camps, and special events

Extreme Performance / Weapons Programs, including training with padded or non‑live martial arts weapons (minor program)
I knowingly and voluntarily assume all risks, both known and unknown, foreseeable or unforeseeable, including risks arising from contact with other participants, even when safety rules and protective equipment are used.
2. SPARRING & STUDENT‑TO‑STUDENT CONTACT
I understand that sparring and partner drills involve intentional physical contact with other students. While safety rules, supervision, and protective gear are used, injuries may still occur, including bruises, sprains, fractures, concussions, or more serious injury.
I expressly assume all risks associated with student‑to‑student contact, and agree that such contact is an inherent part of martial arts training.
3. EXTREME PERFORMANCE / WEAPONS PROGRAM
I understand that participation in the Extreme Performance / Weapons Program involves advanced athletic movement, coordination, speed, and the controlled use of martial arts weapons designed for training purposes.
I acknowledge that despite instruction, supervision, and safety protocols, these activities carry increased risk of injury. I voluntarily assume all such risks on behalf of myself or my child.
4. RELEASE & WAIVER OF LIABILITY
In consideration for being allowed to participate, I hereby release, waive, discharge, and covenant not to sue:
SFL Ninjas, LLC, its owners, instructors, employees, contractors, volunteers, and agents (collectively, the “Released Parties”)
from any and all claims or causes of action arising out of or related to participation in Academy activities, including claims based on the negligence of the Released Parties, to the fullest extent permitted by Tennessee law.
5. INDEMNIFICATION
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or expense (including attorney’s fees) arising from my or my child’s participation, including claims brought by third parties.
6. MEDICAL CONDITION & EMERGENCY CARE
I affirm that the participant is physically able to participate and has no condition that would limit safe participation. I authorize the Academy to obtain emergency medical treatment if necessary and accept financial responsibility for such care.
7. COMMUNICABLE DISEASE RISK
I understand that participation may increase exposure to communicable diseases and voluntarily assume that risk.
8. PHOTO & VIDEO RELEASE
I grant permission for the Academy to use photographs or video recordings of the participant for promotional, educational, or marketing purposes without compensation.
9. RULES, GOVERNING LAW & ENTIRE AGREEMENT
I agree to follow all Academy rules and instructions. This Agreement shall be governed by the laws of the State of Tennessee. If any provision is found invalid, the remaining provisions shall remain in full force. This document constitutes the entire agreement.
